Bleicken Dentistry Referral Program… Thank you!
The entire team at Bleicken Dentistry recognizes that a patient’s referral of a family member or friend to our practice is not only the greatest compliment, but also comes with an important obligation to provide the very best dental care. To thank you for your kind referrals, we have created a Referral Program, the rules and restrictions for which are below.
Every patient who refers another person to our practice will receive their choice of one of the below gift cards. In addition, that person will be entered into a quarterly drawing to win a second appreciation gift with a higher value.
Gift cards are for $25 and include the choice of one of the following:
- The Movies
- A Local Restaurant
- Starbucks
Rules & Restrictions
Our Referral Program officially begins May 5, 2017. Although we sincerely appreciate all referrals made prior to this date, those referrals will not qualify for appreciation gifts or entry into the quarterly drawing.
- There is no limit to the number of gift cards you can earn for referrals that are not immediate family members of each other.
- Earn up to 3 gift cards per family referred. Example: Refer a family of 4, pick 3 gifts cards.
- There is no limit to the number of entries a person can earn for the quarterly drawing. Example: Refer a family of 7, receive 7 entries.
- Referred persons must receive treatment at Bleicken Dentistry. Scheduled appointments, initial consultations, and drop-ins do not qualify as a referred patient.
Prior to, or at the time of setting the appointment with our office, the referred person must either complete the referral source information on the Bleicken Dentistry website intake form, or notify the front desk staff of who referred them. Referral information provided after the initial appointment is scheduled will not qualify for either the gift card or quarterly drawing.
- Gift cards must be picked-up at our office within 45 days of notification.
- By accepting a gift card, recipient agrees to allow Bleicken Dentistry the right to use his or her name and picture on our practice’s online and hardcopy marketing materials.
- Referral gift choices are subject to change based on availability and other factors.
